Willow Lane Acupuncture is a fictional practice, and this is an example notice. A real practitioner's notice must describe what they actually do.

Who I am

I'm Anna Whitlow, and I run Willow Lane Acupuncture on my own from 2 Willow Lane, Kingsmere. I'm responsible for the information in this notice, and I'm registered with the Information Commissioner's Office, registration no. ZDEMO001 (fictional). If you have a question, email anna@example.com.

What I keep

  • Your name and contact details, so I can arrange appointments.
  • What you tell me about your health, and the notes I write after each appointment.
  • A record of what you've paid.

Why

To look after you safely and keep the records my professional body requires. Your health information is special category data; I use it only to provide your care, as a practitioner bound by a duty of confidentiality. I'll never add you to a mailing list unless you ask to be on one.

Who sees it

Only me. My online booking system holds your name, contact details and appointment times on my behalf. I won't share your notes with anyone, including your GP, without your written consent, unless the law requires it or someone is at serious risk of harm.

How long I keep it

Notes are kept for as long as my professional body's guidance and the law require, and then securely destroyed.

Your rights

You can ask for a copy of your notes, ask me to correct anything, or object to how I use your information. If you're unhappy with how I've handled it, please tell me first; you can also complain to the Information Commissioner's Office.

Cookies

This website sets no cookies. It counts visits with a simple counter that doesn't identify you or follow you to other sites, so there's no cookie banner. If you book online, the booking system's own pages may set cookies, which its own notice explains.
