Fenbury Acupuncture Clinic is a fictional practice, and this is an example notice. A real clinic's notice must describe what it actually does.
Who we are
Fenbury Acupuncture Clinic, 41 Market Street, Fenbury, is the data controller for the information in this notice. We're registered with the Information Commissioner's Office, registration no. ZDEMO000 (fictional). Questions go to Marcus Hale at clinic@example.com.
What we collect
- Your name, contact details and date of birth, when you book.
- Health information you give us, and the notes we write at each appointment.
- Records of appointments and payments. Card details are handled by our card provider and never stored by us.
Why, and on what basis
We use your information to provide your care, run appointments and keep the records the law and our professional body require. We rely on needing it to provide the care you've booked. Health information is special category data; we use it to provide health care as practitioners bound by a duty of confidentiality. We never use your information for marketing unless you've separately asked to hear from us, and you can stop that at any time.
Who sees it
Only your practitioner and, where needed, the other practitioner covering your care. Our booking and notes system and our card provider process data on our behalf under contract. Our accountant sees payment records only. We share anything with your GP or anyone else only with your written consent, unless the law requires it or someone is at serious risk of harm.
How long we keep it
Clinical notes are kept for as long as our professional body's guidance and the law require, and then securely destroyed. Payment records are kept for six years for tax purposes.
Your rights
You can ask for a copy of your information, ask us to correct it, or object to how we use it. You can ask us to delete it, though we may need to keep clinical notes for the period above. If you're unhappy with how we've handled your information, tell us first; you can also complain to the Information Commissioner's Office.
Cookies
This website sets no cookies. It counts visits with a simple counter that doesn't identify you or follow you to other sites, so there's no cookie banner. If you book online, the booking system's own pages may set cookies, which its own notice explains.
